Fixed bug when PCMWAVEFORMAT is passed into acmStreamOpen.

Fixed bug where interpolation is done with data past end of buffer. A
proper fix would be to project the last sample based on the previous
two but that requires significant changes.
Added TRACE and WARN where needed.
