Use wine_dlsym instead of dlsym to avoid the glibc dlerror bug.
Call dlsym with RTLD_DEFAULT for symbols that we don't redefine to
work around another glibc bug.

Let the pthread library allocate the stack itself.

Create two separate wine binaries, one using libpthread and one using
kernel threading directly.
Get rid of the remaining HAVE_NPTL ifdefs.
