- rewrote the file code so that streams manage low level file desc

(the code was written the other way around)
- reworked the flag handling code (now the stream._flag is closer to
what MS does)
- simplified temp files handling by using FILE_FLAG_DELETE_ON_CLOSE in
CreateFile
- fixed a couple of bugs
