Change some Dll* functions so they are exported by name like on
Windows.
Update the documentation accordingly.

Don't prefix the functions DllCanUnloadNow, DllGetClassObject and
Dll(Un)RegisterServer with the dll name so that the compiler can check
the prototypes.
