Fix handling of relative state indices in RestoreDC.
Fix return value of SaveDC when writing to an emf.
Before writing the EMR_EOF record we should ensure that we clear the
state stack.

Changed the GDI driver interface to pass an opaque PHYSDEV pointer
instead of a DC structure.
Removed some direct accesses to the DC structure from the drivers.
Got rid the bitmap driver.
