necessary to correctly implement GetClipRgn16 and InquireVisRgn.
SelectVisRgn also expects region in device-relative coordinates.
Adapted the rest of Wine to this coordinate change.

Fixed portability. Adapted some code to make it easier to
integrate the DOS subsystem with the DPMI subsystem, made

Generally moved stuff around a little.
It is now technically possible to load several DOS programs
into the same memory space. Not tested, though.
