Rework clipping so that the PS clip path is only set just before any
graphics output event. Doing it this way means we don't ever need to
call initclip which is a Good Thing.

Changed the GDI driver interface to pass an opaque PHYSDEV pointer
instead of a DC structure.
Removed some direct accesses to the DC structure from the drivers.
Got rid the bitmap driver.
