Eric Pouech 8d108a2167 Removed fixed size array to store specific data (now using 
mci(Set|Get)DriverData to store specific data).
Make use of default debugtools.h channel.

Eric Pouech 6ba9d96e61 Changed way of allocation open mci drivers (array => list). 
First valid ID for mci devices is now 1 (some progs were testing this
value).
Started implementing mci command table resources.
Handled closing on MCI_ALL.

Ove Kaaven 1a22964240 Use service thread for "true" multimedia timers. 
Removed implementation of fake multimedia timers.
Set minimum resolution to 10 ms (the timing resolution of the Linux
kernel). Fixed a couple of flaws.
