msvcrt: Fix handling of 'h' prefix in swprintf.

- '%hs' in widechar string functions is wrongly recognized as
  a widechar string. Fix this by always checking the prefix first.
- add testcase
- remove code duplication in %c/%s handling by moving code into a function.
